About the position

The Home Health RN Case Manager is responsible for planning, organizing, and directing home health care services for patients in the NE Portland, Gresham, and Troutdale areas. This role involves conducting comprehensive assessments, developing individualized care plans, and providing professional nursing care in accordance with state regulations. The position offers flexible work schedules and opportunities for professional growth and development.

Responsibilities

  • Plan, organize, and direct home health care services.
  • Conduct initial, comprehensive, and ongoing assessments of patients and their living situations.
  • Provide professional nursing care utilizing the nursing process as defined in the state Nurse Practice Act.
  • Establish individualized plans of care in collaboration with certifying medical providers.
  • Regularly re-evaluate patient and family needs and revise care plans as necessary.
  • Develop care plans that include interventions and goals, involving patients and families in the planning process.
  • Administer medications and treatments as prescribed by medical providers.
  • Provide health care instructions to patients as appropriate.
  • Act as Case Manager for assigned caseload, coordinating patient care.

Requirements

  • Graduate of an accredited school of nursing.
  • 1-2 years of recent acute care experience in an institutional setting.
  • 1-2 years of recent experience in home health preferred.
  • Currently licensed as a Registered Nurse in the state of practice.
  • Possess and maintain CPR certification.
  • Licensed driver with an insured automobile in good working order.
  • Excellent observation, verbal and written communication skills.
  • Problem-solving skills and basic math skills.
  • Self-directed and able to work with minimal supervision.
  • Capable of prolonged walking or standing, lifting, positioning, and transferring patients.
