About the position
The Home Health Registered Nurse (RN) at Tufts Medicine Care at Home is responsible for managing and delivering comprehensive home health services to clients in their residences. This role emphasizes assessments, interventions, and supportive care, with a focus on avoiding hospitalization. The RN will work autonomously, build rapport with patients and families, and collaborate with healthcare teams to ensure safe and efficient patient care.
Responsibilities
- Assess patients' physical and psychosocial needs in a sensitive manner following established nursing standards.
- Establish realistic goals and develop treatment plans in cooperation with patients, families, and healthcare team members.
- Adapt to new situations without negatively affecting work performance.
- Collaborate with patients, families, and other healthcare providers in care planning and discharge processes.
- Complete physicians' orders and OASIS documentation for assigned patients.
- Reconcile medications with patients and physicians consistently.
- Manage assigned caseload effectively within a team model of care delivery.
- Establish a daily work plan based on patient and family priorities.
- Triage patient visits, messages, and phone calls according to urgency.
- Coordinate care and discharge planning during case conferences.
Requirements
- Massachusetts RN Licensure.
- Valid state issued Driver's License.
- One year of acute medical/surgical nursing experience.
- Cardiopulmonary Resuscitation (CPR) Certification.
