About the position

The Home Health RN Case Manager is responsible for providing comprehensive nursing care to patients in their homes, ensuring that their health needs are met through effective case management. This role involves assessing, planning, implementing, coordinating, monitoring, and evaluating patient care services in collaboration with healthcare professionals and resources. The RN Case Manager acts as a patient advocate and resource for field staff, ensuring quality outcomes and adherence to treatment plans.

Responsibilities

• Conduct initial evaluation visits to clients and develop care plans in accordance with physician directives.
• Schedule re-certification and follow-up visits 5 days prior to the next certification period.
• Receive patient referrals and assign RNs for admissions in the absence of an intake nurse.
• Communicate with field staff, physicians, and other involved agencies to ensure coordinated patient care.
• Report any unusual conditions or complaints concerning patients to the Clinical Director.
• Demonstrate competency in nursing skills such as venipunctures, infusion therapy, wound care, and aseptic technique.

Requirements

• Current RN License in the State of Texas.
• 1-2 years of experience in home health or acute care setting.
• Minimum of one year of nursing experience required.
• Ability to effectively communicate with physicians, patients, and caregivers.

Nice-to-haves

• Experience with Wellsky (Kinnser) software.
