About the position

The RN Case Manager at Alpine Home Health Agency is responsible for providing exceptional home health care to pediatric patients, ensuring they receive personalized and high-quality care in their homes. This role involves conducting assessments, developing care plans, educating families, and coordinating with healthcare teams to support children with long-term illnesses or genetic disorders.

Responsibilities

• Conducting initial assessments and developing personalized care plans.
• Regularly reassessing patients' needs and updating care plans as required.
• Educating patients and their families on care techniques and medication management.
• Monitoring patient progress and maintaining accurate documentation.
• Evaluating home environments for safety and recommending necessary modifications.
• Ensuring compliance with healthcare regulations and guidelines.
• Participating in case conferences and coordinating with the healthcare team.

Requirements

• Valid RN license in California.
• At least 2 years of RN experience.
• CPR/BLS certification.
• Strong clinical judgment and critical thinking abilities.
• Excellent communication skills.
• Proficiency in electronic health record systems.
• Recent physical exam (within the last year).
• Negative TB test (within the last year).
• Malpractice insurance (NSO).

Nice-to-haves

• Experience in patient monitoring.
• Knowledge of Spanish.
