About the position
The Home Health Registered Nurse (RN) position is a contract role based in Vancouver, WA, requiring a commitment of 13 weeks. Responsibilities
- Complete thorough patient assessments and OASIS documentation
- Develop individualized care plans based on patient needs
- Provide direct nursing care, including medication management and wound care

- Educate patients and families on health practices and self-care techniques
Requirements
- Current RN license in Washington
- Proficiency in OASIS and experience with Homecare Homebase required
- 2 years of Home Health experience preferred
- Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
