About the position
The RN Visit Nurse at Lifespark is responsible for planning, organizing, and directing home care services for seniors. This role emphasizes community health education and requires a professional nurse to build resources within the community to meet the needs of individuals and families in their homes. The position offers a flexible schedule with three 12-hour shifts per week, allowing for a work-life balance while providing essential care to seniors.
Responsibilities
- Plan, organize, and direct home care services for clients.
- Provide nursing care with an emphasis on community health education.
- Build resources within the community to meet client needs.
- Supervise caregivers and ensure quality of care.
- Maintain accurate records and documentation of patient care.
Requirements
- One to two years of recent acute care experience in an institutional setting.
- One year of home health care experience preferred.
- Experience in customer service or direct member contact field preferred.
- Experience supervising caregivers.
- Graduate of an accredited school of nursing.
- Current Minnesota Nursing License or Minnesota Licensed Practical Nurse.
- Must pass a MVR check based on Lifespark company policy.
