About the position

The Home Health Scheduler plays a key role in ensuring timely and efficient coordination of home health services for patients. This position involves scheduling, intake, and ensuring that all necessary resources are in place to provide high-quality, continuous care. The Scheduler will report to the Director of Operations and work closely with the Director of Nursing, field staff, and management to optimize care delivery and ensure compliance with scheduling protocols.

Responsibilities

  • Manage and schedule patient visits for home health staff, including nurses, therapists, and home health aides.
  • Serve as a primary point of contact for patients and caregivers to confirm and adjust appointments.
  • Keep detailed and accurate records of patient appointments, visit histories, and provider schedules.
  • Identify and resolve scheduling conflicts promptly, adjusting visits as needed to meet both patient and staff requirements.
  • Monitor the status of patient care and ensure that follow-up appointments are scheduled in a timely manner.
  • Ensure all scheduling practices comply with applicable healthcare regulations, including HIPAA and patient privacy requirements.
  • Work closely with clinical staff to understand patient care needs and update the care team on patient status or any changes in the care plan.
  • Respond to phone calls, emails, and other inquiries related to scheduling and appointment requests.

Requirements

  • High school diploma or equivalent required; Associate's or Bachelor's degree in healthcare administration, business, or a related field preferred.
  • Previous experience in a healthcare setting, preferably in home health or a similar field, is strongly preferred.
  • Prior experience with medical scheduling software or electronic health records (EHR) is an advantage.
  • Strong organizational skills with the ability to manage multiple tasks and prioritize effectively.
  • Excellent communication skills, both verbal and written.
  • Attention to detail and accuracy in recordkeeping.
  • Ability to work independently and as part of a team.
  • Strong problem-solving skills and the ability to remain calm under pressure.
  • Proficient with computers and scheduling software (e.g., Microsoft Office, electronic scheduling systems).
  • Knowledge of healthcare regulations, especially in home health, is a plus.
