Home Health Social Worker Part Time

About the position

As a social worker, you will provide preventive, educational, and evaluative and treatment services to meet the psychosocial needs of patients and their families, to help them improve, restore, and maintain their maximum level of coping. You will utilize community resources and the members of the interdisciplinary team to aid in this process.

Responsibilities

• Provide direct social work services and counseling to patients and families.
• Facilitate placements and monitor patient's adjustment.
• Complete assessments of each patient and review home health services.
• Address financial concerns for the family.
• Develop and utilize community resources and maintain working relationships with community agencies.
• Provide education and in-services to contract agencies.

Requirements

• Effective communication and interpersonal skills
• Strong leadership and influential skills
• High sense of integrity
• Ability to travel to patient homes
• Ability to effectively and compassionately present information to patients and families
• Knowledge of Microsoft Office programs

Nice-to-haves

• Master of Social Work (MSW) degree from an accredited school of social work (preferred)
• Baccalaureate degree in social work, psychology, sociology, or related field with 1 year of social work experience in a healthcare setting
