About the position

The Home Health Registered Nurse (RN) position at Interim HealthCare involves providing comprehensive nursing care to patients in their homes across Dorchester, Charleston, and Berkeley Counties. The role emphasizes patient assessment, care planning, and collaboration with interdisciplinary teams to ensure high-quality patient outcomes. The RN will also supervise paraprofessional staff and participate in quality improvement initiatives, all while enjoying a supportive work environment that values work/life balance and career growth.

Responsibilities

• Participates in ongoing interdisciplinary assessment of the patient.
• Completes initial and ongoing comprehensive assessments of the patient's needs.
• Participates in the development and evaluation of the plan of care in partnership with the patient, representative, and caregiver(s).
• Clarifies mutually agreed upon goals with the patient and incorporates these into the plan of care/service plan.
• Provides services ordered by the physician as indicated in the plan of care.
• Counsels and educates the patient and family in meeting healthcare-related needs.
• Initiates appropriate preventative and rehabilitative nursing procedures.
• Refers to other services as needed.
• Monitors and evaluates outcomes of patient care.
• Communicates with appropriate staff regarding changes in the patient's needs.
• Supervises paraprofessional staff.
• Effectively communicates with other members of the interdisciplinary healthcare team to promote coordination of patient care.
• Communicates with all physicians involved in the plan of care and other healthcare practitioners related to the current plan of care.
• Participates in Quality Assessment and Performance Improvement (QAPI) program.
• Participates in in-services or training, as requested.

Requirements

• Graduate of an accredited school of nursing and current RN License in SC or compact license.
• 2 years' experience as a nurse with 1 year of experience in home health.
• Proficient computer skills and workplace software knowledge.
• Ability to work with people, function well under stress, use clinical judgment, be creative, and accept challenges.
• Able to effectively deal with multiple changes based on patient needs and scheduling.
• Clinical proficiency in nursing, strong communication skills, and compassion.
• CPR Certification.
• Valid driver's license and proof of insurance.
