About the position

As a Home Infusion Nurse (RN) with Accredo, you will embark on a nursing career experience that is both fulfilling and impactful. This role is designed for those who are passionate about patient care and wish to make a significant difference in the lives of individuals with complex health conditions. You will travel to patients' homes in the northeast Houston area, including Magnolia, Cypress, Katy, and NE Houston, to administer critical infusion drugs. However, your responsibilities extend beyond just medication administration; you will have the opportunity to develop meaningful relationships with your patients, ensuring they feel supported and empowered throughout their treatment journey. In this position, you will work independently, making informed decisions that prioritize the best outcomes for your patients. You will be responsible for coaching patients, focusing on their overall well-being, and collaborating with pharmacists and the Therapeutic Resource Center (TRC) to meet their needs effectively. As the single point of contact for your patients, you will provide regular updates on their status, document all interactions, and manage follow-up care. Your role will require strong IV insertion skills and the ability to handle various situations with confidence and adaptability, supported by your team and leadership. This position is not just about delivering care; it’s about challenging yourself to grow as a nurse every day. You will be expected to maintain a positive attitude and demonstrate flexibility in your work schedule, which may include occasional evening or weekend visits as needed. Your commitment to your craft will be essential in navigating the complexities of patient care in a home setting, ensuring that you provide the highest level of service and support to those you serve.

Responsibilities

  • Travel to patient homes to administer IV infusion medications autonomously.
  • Develop meaningful relationships with patients to empower them in their care.
  • Document all interactions and communications with patients, including assessments and treatments.
  • Collaborate with pharmacists and the Therapeutic Resource Center to meet patient needs.
  • Provide response management and follow-up care for patients.
  • Act as the single point of contact for regular updates on patient status.
  • Challenge yourself to improve as a nurse and handle difficult situations with support.

Requirements

  • Registered Nurse (RN) license in good standing in the state of residence.
  • BSN degree with 2+ years of relevant RN experience; or ASN degree with 5+ years of relevant RN experience; or Nursing Diploma with 8+ years of relevant RN experience.
  • 1+ years as a specialty RN in a critical care, acute, or home healthcare environment.
  • Strong IV insertion skills.
  • Valid driver's license.
  • Willingness to travel within an assigned geographic area.
  • Ability to work Monday-Friday, day shift, with occasional evening or weekend visits as needed.
  • Flexibility to work alternate shifts on short notice and be on call for field visits.
