About the position

As a Home Infusion Nurse (RN) at Accredo, you will embark on a nursing career experience that is truly unique. This role is not just about delivering critical infusion drugs to patients in their homes; it is about building meaningful relationships with them. In this role, you will be responsible for empowering your patients, acting as a coach to help them achieve their best health outcomes. You will collaborate with Pharmacists and the Therapeutic Resource Center (TRC) to ensure that all patient needs are met. Your responsibilities will include traveling to patient homes to administer IV infusion medications autonomously, managing responses, and providing follow-up care. You will serve as the single point of contact for regular updates on patient status, utilizing a tablet to document all interactions, assessments, treatments, and disease progression. This position challenges you to continuously improve as a nurse. You will be expected to demonstrate a commitment to your craft, confidence in your abilities, and a positive attitude, all while adapting to the various challenges that may arise. Support from your leader and team will be available to help you navigate even the most difficult situations, ensuring that you can provide the highest level of care to your patients.

Responsibilities

  • Travel to patient homes to administer IV infusion medications autonomously.
  • Provide response management and follow-up care to patients.
  • Document all interactions and communication with patients, including assessments, treatments, and disease progression.
  • Act as the single point of contact for regular updates on patient status.
  • Collaborate with Pharmacists and the Therapeutic Resource Center (TRC) to meet patient needs.
  • Empower patients by coaching them to achieve their best health outcomes.
  • Adapt to various challenges and demonstrate a commitment to nursing excellence.

Requirements

  • Registered Nurse (RN) license in good standing in the state of residence.
  • BSN degree with 2+ years of relevant RN experience; or ASN degree with 5+ years of relevant RN experience; or Nursing Diploma with 8+ years of relevant RN experience.
  • 1+ years as a specialty RN in a critical care, acute, or home healthcare environment.
  • Strong IV insertion skills.
  • Valid driver's license.
  • Willingness to travel within an assigned geographic area.
  • Ability to work Monday-Friday, day shift, with occasional evening or weekend patient visits as needed.
  • Flexibility to work alternate shifts on short notice and be on call for field visits as determined by business need.
