About the position

The position involves dispensing medications and providing pharmaceutical care in a pharmacy setting, ensuring compliance with regulations and company policies. The role focuses on improving health outcomes by connecting patients with necessary medications and resources, while also contributing to health equity on a global scale. The pharmacist will work collaboratively with healthcare teams to monitor patient therapy and ensure the safety and effectiveness of pharmaceutical care.

Responsibilities

  • Dispense unit doses, specialty pharmaceuticals, injectables, IV, enteral and related supplies following state and federal regulations and company policies.
  • Compound medications using appropriate components, supplies, and sterile/ventilated environment.
  • Process, pack and ship outgoing pharmacy orders, ensuring drug, related supplies, and patient collateral are included.
  • Perform drug/supply ordering and inventory/warehouse duties, including proper storage of drugs and supplies and appropriate refrigeration of perishable products.
  • Review medication orders and evaluate appropriateness and completeness of orders considering the special needs of the specific age group served.
  • Verify all prescriptions not directly received from the prescriber or physician's agent.
  • Provide oversight of the safety, appropriateness, and effectiveness of the pharmaceutical care provided to patients.
  • Assess the patient at the initiation of drug therapy, develop a patient care plan and drug monitoring regimen, and monitor the patient according to the care plan throughout the course of therapy.
  • Communicate with the patient, physician, nursing staff, and other members of the health care team regarding patient status and response to therapy.
  • Compound intravenous and specialty pharmaceuticals in an aseptic fashion according to established guidelines and company policy.
  • Ensure compliance with all FDA, DEA, State Board of Pharmacy, OSHA, and other regulatory standards.
  • Provide indirect oversight of pharmacy technician and pharmacy support staff.
  • Monitor and verify all prescription orders upon creation and dispensing.
  • Interpret provider prescription and create order, access, input and retrieve prescription and medical/lab information from computer system.
  • Contact members/patients to validate data, resolve rejected claims, and consult on medication issues.

Requirements

  • BS in Pharmacy or PharmD degree
  • Current, unrestricted Pharmacist license in the state of Oregon
  • Solid drug product and clinical pharmacy knowledge
  • Knowledgeable in various infusion related therapies
  • Proficient with Microsoft Office
  • Ability to fill medication orders accurately and in a timely manner
  • Ability to work a flexible schedule (8-hour shifts between hours of 7am-7pm weekdays with rotating weekends) and rotate being on call based on business needs

Nice-to-haves

  • Nutritionally Certified Pharmacist with Home Care or Home Infusion experience
  • Sterile compounding experience
  • CPR+ software experience
