About the position

As a Home Provider with Mosaic at Home, you will share your home with individuals with disabilities, providing essential support and fostering independence in daily living tasks. This role is designed to create a safe and healthy environment while assisting individuals with various activities, including cooking, cleaning, and medical appointments. Responsibilities

• Provide support for tasks such as cooking, cleaning, laundry, budgeting, and decision-making.
• Assist with skill acquisition and medication administration.
• Accompany individuals to medical appointments and support their overall health and well-being.
• Maintain daily documentation via a web-based case management application (THERAP).
• Facilitate fun and leisure experiences for individuals, promoting community inclusion and socialization.
• Ensure compliance with all state and agency policies and procedures.
• Communicate effectively with team members as needed.
• Support the physical needs of the individual served, including transportation.

Requirements

• Must be a resident of the state of Nebraska.
• Must be at least 18 years of age.
• Valid driver's license, car insurance, and current vehicle registration are required.
• Active home or renters insurance is necessary.
• Own or rent your own home, apartment, condo, or townhouse.
• Reliable computer and internet access is essential.
• Ability to pass State/Federal and APS/CPS background checks.

Nice-to-haves

• Experience in caregiving or support roles is preferred but not required.
• Ability to provide assistance with bathing, toileting, feeding, and behavioral support as needed.
• Experience caring for individuals with complex physical or medical needs.
