About the position

As a Host Home / Shared Living provider in Nebraska, you will create a safe and healthy home environment for individuals with disabilities while fostering their independence and life skills. This role involves providing support for daily living tasks and participating in community activities, tailored to the individual needs of those you support. Responsibilities

• Provide support for tasks such as cooking, cleaning, laundry, budgeting, and decision-making.
• Assist with skill acquisition and medication administration.
• Accompany individuals to medical appointments and support their overall health and well-being.
• Document daily activities via a web-based case management application (THERAP).
• Facilitate fun and leisure experiences for individuals, promoting community inclusion and socialization.
• Ensure compliance with all state and agency policies and procedures.
• Communicate effectively with team members as needed.
• Support the physical needs of the individual served, including transportation.

Requirements

• Must be a resident of Nebraska.
• Must own or rent your own home/apartment/condo/townhouse.
• Must be at least 18 years of age.
• Valid driver's license, car insurance, and current registration are required.
• Active home/renters insurance is necessary.
• Reliable computer and internet access is required.
• Ability to pass State/Federal and APS/CPS background check.

Nice-to-haves

• Experience in caregiving or support services is preferred but not required.
• Ability to provide assistance with bathing, toileting, feeding, and behavioral support.
• Experience caring for individuals with complex physical/medical needs.
