About the position

The Host Home / Shared Living provider role with Mosaic at Home offers a meaningful opportunity to support individuals with disabilities in a home environment. Providers will share their home with a person with a disability, ensuring a safe and healthy living space while assisting with daily living tasks and fostering independence. This position allows for a flexible income based on the level of support provided, with an average annual income of $33,000, which is federally tax-free.

Responsibilities

• Provide support for tasks such as cooking, cleaning, laundry, budgeting, and decision-making.
• Assist with skill acquisition and daily living activities.
• Administer medication and support medical appointments.
• Document daily activities and progress using a web-based case management application (THERAP).
• Facilitate community inclusion and socialization opportunities for the individual.
• Ensure compliance with all state and agency policies and procedures.
• Communicate effectively with team members as needed.
• Support the physical needs of the individual served, including transportation.

Requirements

• Must be a resident of Illinois.
• Must be at least 21 years of age.
• Own or rent your own home, apartment, condo, or townhouse.
• Transportation, valid driver's license, car insurance, and current registration are required.
• Active home or renters insurance is necessary.
• Reliable computer and internet access is required.
• Ability to pass State/Federal and APS/CPS background checks.

Nice-to-haves

• Experience in caregiving or related fields is preferred but not required.
• Ability to provide support for complex physical or medical needs.
