About the position
The Homecare Hospice Aide position at Hospice of the Piedmont involves providing personal care and home management tasks to patients in their residences under the supervision of a Registered Nurse (RN) Case Manager. The role emphasizes compassionate care, effective communication, and collaboration with interdisciplinary teams to achieve patient care goals.
Responsibilities
- Provide personal hygiene, elimination, feeding, and mobility assistance to patients according to the nurse aide care plan.
- Report changes in the patient's condition and concerns to nursing staff in a timely manner.
- Participate in interdisciplinary team conferences as necessary.
- Document patient care accurately and in accordance with agency policies and procedures.
- Manage time, people, medical supplies, and technology effectively.
- Establish and adhere to a timeframe for patient visits in coordination with patients, families, and facility staff.
- Maintain a clean, safe, and comfortable environment for the patient.
- Teach and reinforce home safety, energy conservation, and sanitation practices.
- Perform light housekeeping tasks and assist with meal preparation.
- Demonstrate excellent communication skills and build successful interpersonal relationships with patients and families.
- Comply with organizational nurse aide standards and participate in staff training and education programs.
Requirements
- 1 year of experience as a nurse aide (CNA).
- High School diploma or GED.
Nice-to-haves
- Certified Hospice & Palliative Nursing Assistant (CHPNA) certification.
