About the position

The Mental Health Case Manager for the Homeless Services Team at Johnson County Government plays a crucial role in providing community-based support to adults experiencing severe mental illness. The Case Manager works collaboratively with other providers and community agencies to ensure effective service delivery and support systems for clients.

Responsibilities

• Conduct assessments of individuals' backgrounds and circumstances to identify needs and determine service eligibility.
• Develop plans to address the needs of individuals in single or multiple areas of focus.
• Network with community agencies to research and coordinate resources and services for clients.
• Encourage the establishment and maintenance of support systems for clients.
• Consult and collaborate with other providers in the development, implementation, and review of service plans.
• Provide a variety of services for a designated caseload of individuals with ongoing needs.
• Ensure services provided are consistent with individual plans.
• Initiate and attend individual and/or group meetings and provide services in a community-based setting.
• Provide transportation for individuals as identified in their plans.
• Organize and facilitate skill development and support groups.
• Complete and submit documentation regarding services provided.
• Evaluate and document individuals' progress according to measurable goals.
• Review the quality and effectiveness of services provided.

Requirements

• Bachelor's degree in Human Services or related field.
• Valid driver's license with an acceptable driving record.
