About the position

The Mental Health Case Manager for the Homeless Services Team at Johnson County Government plays a crucial role in providing community-based services to adults experiencing severe mental illness. This position involves conducting assessments, developing individualized service plans, and coordinating with various community agencies to ensure comprehensive support for clients. The Case Manager will work within a strengths-based framework, focusing on empowering individuals and facilitating their access to necessary resources and services.

Responsibilities

• Conduct assessments of individuals' backgrounds and circumstances to identify needs and determine service eligibility.
• Develop plans to address the needs of individuals in single or multiple areas of focus.
• Network with community agencies to research and coordinate resources and services for clients.
• Encourage the establishment and maintenance of support systems for clients.
• Consult and collaborate with other providers in the development, implementation, and review of service plans.
• Provide a variety of services for a designated caseload of individuals with ongoing needs.
• Ensure services provided are consistent with individual plans.
• Initiate and attend individual and/or group meetings and provide services in a community-based setting.
• Provide transportation for individuals as identified in their plans.
• Organize and facilitate skill development and support groups.
• Complete and submit documentation regarding services provided and evaluate individuals' progress according to measurable goals.
• Review the quality and effectiveness of services provided.

Requirements

• Bachelor's degree in Human Services or a related field.
• Valid driver's license with an acceptable driving record.
• Reference/background screening as required by Johnson County Government.
