About the position

The Hospice Admissions RN plays a crucial role in providing compassionate care to patients and their families during the admission process into hospice services. This position involves assessing patient needs, developing care plans, and collaborating with healthcare teams to ensure the highest quality of care. The role emphasizes flexibility in scheduling to allow for personalized patient interactions and work-life balance.

Responsibilities

  • Initiates a plan of care following assessment of the patient, collaborating with the patient, family, caregiver, and/or facility staff.
  • Assesses patient's immediate needs and symptoms, collaborating with caregivers, physicians, and pharmacists to manage those symptoms.
  • Documents all medications, the plan of care, and the Hospice Initial Assessment (OASIS for Home Health).
  • Sets up the ECF/hospice chart in facilities for ECF patients.
  • Completes Matrix charting in the home or facility, including admitting patient, medication entry, and starting of admission assessment.
  • Sends NEW ADMIT email report of admission to appropriate services sight email group, uploads consents, legal documents, emergency plan, POST into Matrix.

Requirements

  • Licensed RN in the State of Indiana
  • Experience preferred in hospice care, home health, oncology, and/or pain and symptom management
  • Current CPR certification
  • Must have and maintain a valid driver's license, good driving record, and auto accident insurance
