About the position

The Hospice Admissions Nurse Liaison at Oasis Hospice & Palliative Care, Inc. is responsible for providing compassionate care to families in Oak Park, IL. This role involves conducting assessments to determine hospice needs, explaining hospice philosophy to patients and families, and coordinating care with physicians. The position requires strong communication skills and the ability to work collaboratively within an interdisciplinary team.

Responsibilities

  • Complete initial and comprehensive assessments of patients and families to determine hospice needs.
  • Meet with patients and families to explain hospice philosophy.
  • Initiate communication and obtain necessary physician orders for patient care with attending physicians and hospice medical directors.
  • Provide a complete physical assessment and history of current and previous illnesses.
  • Utilize all elements of the nursing process to provide professional nursing care.
  • Assess and evaluate patient's status by writing and initiating a plan of care, including the patient and family in the planning process.
  • Participate in interdisciplinary team meetings to develop, review, and/or revise each patient's plan of care.
  • Participate in the on-call process as stated in agency policy.
  • Maintain regular communication with the RN Clinical Manager to review case load.
  • Administer medications and treatments as prescribed by the physician in the physician's plan of care.

Requirements

  • Current RN licensure in the State of Illinois
  • CPR certification
  • Bachelor's or Associate's degree
  • One (1) year of hospice care experience preferred but not required
  • Excellent observation, verbal and written communication skills
  • Licensed driver with an insured automobile in good working order

Nice-to-haves

  • Bilingual in English and another language
  • Experience in long-term care or nursing home settings
