About The Position

The Hospice and Home Health Supervisor at Mercy San Juan Medical Center is responsible for overseeing the clinical services of the Home Health and Hospice departments, ensuring high-quality patient care and compliance with regulatory standards. This role involves collaboration with various healthcare professionals to assess patient eligibility and appropriateness of care, while also managing operational costs and promoting quality outcomes.

Requirements

• Minimum of 2 years of management experience within the last 5 years in a Home Care agency, primary care clinic, or health facility.
• Sufficient background knowledge and expertise in clinical decision making for the assigned patient population.
• Current CA RN licensure, active CA Driver's License, auto insurance, and AHA BLS.

Nice To Haves

• Leadership experience specific to Home Health and Hospice.
• Bachelor's or Master's degree in health or business related field.
• Past Hospice experience.

Responsibilities

• Supervising all clinical services in Home Health & Hospice to meet department specific metrics.
• Ensuring compliance with State and Federal licensing and regulatory requirements, ACHC Accreditation, and practice standards.
• Consulting with the MHH/MH Operations Team, Hospice Medical Director, Interdisciplinary Team, and primary physician to assess patient eligibility and care appropriateness.
• Providing oversight to assure continued appropriateness of hospice patients throughout their stay.
• Representing the agency at internal and external meetings to address Home Health & Hospice issues.
