About the position

The Hospice Care Coordinator at Diamond Hospice is responsible for providing administrative support and coordination of care within the hospice environment. This role requires excellent organizational skills, attention to detail, and the ability to communicate effectively with both clients and staff. The coordinator will manage various clerical tasks while ensuring compliance with regulations and maintaining high standards of customer service.

Responsibilities

• Answer calls in a professional manner
• Order supplies and Durable Medical Equipment (DME)
• Maintain employee personal files for HR
• Work closely with clinical staff and other departments
• Respond to client inquiries and maintain good customer relations
• Perform clerical duties such as word processing, scanning, faxing, photocopying, and emailing
• Assist other department staff as needed

Requirements

• Fluency in English
• Basic understanding of HIPAA regulations
• 1 year of experience in home health (preferred)
• Associate degree (preferred)
• Excellent communication skills
• Ability to multi-task and follow directives accurately
