The Registered Nurse - Hospice is responsible for providing care, comfort, and support to terminally ill patients. This role involves assuming responsibility and accountability for the delivery of hospice patient care in various settings. The Registered Nurse demonstrates the ability to make clinical judgments effectively and efficiently, with supervision. Critical thinking and performance ability are essential in the coordination of patient care, ensuring that nursing standards are consistently met. The Registered Nurse is accountable for managing patient care and assisting others in this process, ensuring a high standard of care is maintained throughout the patient's experience. In this position, the Registered Nurse will utilize the nursing process to assess, plan, implement, and evaluate patient care. This includes performing focused interviews to identify specific patient needs, assessing signs and symptoms that indicate physiological and psychosocial changes in the patient's condition, and collecting, analyzing, and interpreting data from healthcare team members. The nurse will document actual and/or potential nursing diagnoses and the patient's plan of care, which includes identified nursing diagnoses, expected patient outcomes, and selected nursing interventions. The Registered Nurse will identify and prioritize actions based on patient care requirements, perform interventions according to identified priorities, and ensure compliance with hospital policies and patient care outcome standards. Creating a safe, comfortable, and therapeutic environment for patients is paramount. The nurse will also initiate and evaluate patient and/or family education, providing guidance and support as needed. The plan of care will be revised according to evaluations and changes in the medical plan, ensuring that the patient's response to nursing and other interventions is documented accurately. The role requires monitoring the safety and effectiveness of the environment and equipment, advising grieving family members on coping techniques, and performing other duties as assigned or necessary.
