About the position
The Care Transition Coordinator at Baptist Hospice of Batesville is responsible for ensuring a smooth transition for patients moving from a facility to hospice care. This role emphasizes building trusting relationships with patients and their families, participating in interdisciplinary team meetings, and contributing to the agency's growth strategies.
Responsibilities
- Develop trusting relationships as an end-of-life care expert.
- Be valued and respected by patients and their families.
- Participate in bi-weekly IDG meetings to give updates regarding accounts, customer needs, and progress towards agency growth strategies.
Requirements
- One year of hospice experience or one year of hospital case management experience.
- Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) or Social Worker (SW) licensure in state of practice.
- Excellent organizational skills.
- Excellent verbal and written communication skills.
