About the position

The Certified Hospice Aide is a vital member of the interdisciplinary team, providing essential personal care and support to patients in a hospice setting. This per diem position involves working under the supervision of a registered nurse to ensure the comfort and well-being of patients, while also maintaining effective communication with their families. The role requires a compassionate approach to care, with a focus on patient observation and documentation of care provided.

Responsibilities

  • Perform personal care needs after given a patient's assignment and attend to requests promptly.
  • Provide positive communication techniques to patients and families, ensuring confidentiality and ethical care.
  • Meet safety needs of patients and use equipment safely and properly.
  • Give personal care including baths, back rubs, oral hygiene, shampoos, and changing bed linen as assigned.
  • Assist in dressing and undressing patients as assigned.
  • Plan and prepare nutritious meals, including shopping, as assigned.
  • Assist in feeding the patient as assigned.
  • Take and record vital signs such as temperature, pulse, respiration, and blood pressure when ordered.
  • Provide proper care and observation of patient's skin to prevent breakdown of tissue.
  • Assess and report on patient's condition and significant changes to the Case Manager.
  • Assist in ambulation and exercise as instructed by the hospice nurse or therapist.
  • Provide normal range of motion and transfers/positioning as directed.
  • Assist with self-administered medications as allowed by state regulations.
  • Offer and assist with bedpans and urinals.
  • Provide assistance with light laundry needs as assigned.
  • Perform range of motion and other simple procedures as ordered.
  • Provide respite for patient's family/caregiver when on-site, as appropriate.
  • Keep patient's living area clean and orderly, as assigned.
  • Adhere to the organization's documentation and care procedures.

Requirements

  • Must have a CNA (Certified Nursing Assistant) and HA (Home Health Aide) license.
  • Completed a minimum of 75 hours of classroom and supervised practical training.
  • At least 18 years of age.
  • Ability to read and follow written instructions and document care given.
  • Understanding of hospice philosophy and comfort in providing specialized care to the terminally ill.
  • Satisfactory references from previous employers.
  • Self-directing with the ability to work with little direct supervision.
  • Secure with issues of death/dying and provide a calm manner in a patient's home.
  • Empathy for the needs of the ill, injured, frail, and impaired.
  • Current CPR certification.
  • Good oral and written communication skills.
  • Good organizational skills.
