About the position

The Hospice Chaplain provides spiritual and bereavement care to hospice patients and their caregivers, primarily working in patient homes across Tomball and Kingwood locations. The role involves performing spiritual and bereavement assessments, counseling, and referrals to meet the needs of patients and their families.

Responsibilities

  • Provide spiritual and bereavement assessments for hospice patients and their families.
  • Perform counseling and referrals to meet the spiritual needs of patients.
  • Attend interdisciplinary team meetings and participate in patient care planning.
  • Collaborate with the interdisciplinary team to promote coordination of patient and bereavement care.
  • Initiate and update bereavement care plans for identified families.
  • Provide continued bereavement support to hospice families as needed.
  • Meet mandatory continuing education requirements of the agency.
  • Demonstrate commitment to professional growth and competency in the field.

Requirements

  • Master's degree in Divinity preferred.
  • Valid Driver's License required.
  • Experience in a spiritual or religious organization.
  • One year experience as a Spiritual Professional in a healthcare institution preferred.
  • Experience or training in grief/loss counseling.
  • Working knowledge of community spiritual resources.
  • Good interpersonal skills.
