About the position

The Hospice Chaplain at Uni Hospice is responsible for providing spiritual care and support to patients and their families during end-of-life transitions. This per-diem role emphasizes compassion, teamwork, and professionalism, requiring the chaplain to conduct spiritual assessments, participate in care planning, and offer direct support while collaborating with an interdisciplinary team.

Responsibilities

  • Conduct thorough spiritual assessments for patients and their families within the hospice program, ensuring timely provision of necessary services.
  • Participate in crafting personalized care plans through comprehensive evaluations addressing spiritual needs.
  • Offer direct spiritual support to both patients and their caregivers.
  • Act as a bridge and support system for community chaplains and spiritual counselors.
  • Keep detailed records of spiritual care services for quality assurance and to inform program development and policy updates.
  • Document all direct spiritual services and maintain communication with community spiritual leaders.
  • Engage in patient care conferences and interdisciplinary group meetings, contributing as an integral team member.
  • Provide expert advice, education, and support on spiritual matters to the interdisciplinary team.
  • Take on additional responsibilities and duties as needed to support the hospice program's goals and services.

Requirements

  • MUST have great communication skills and have prior hospice experience.
  • Graduate from an accredited seminary or school of theology, ensuring a solid foundation in religious studies and pastoral care.
  • Proficient in working collaboratively within an interdisciplinary team, contributing to a cohesive group dynamic.
  • Comfortable engaging in interfaith settings, demonstrating respect and openness to diverse religious practices and beliefs.
  • Skilled in embracing and respecting various lifestyles, cultures, beliefs, and values, fostering an inclusive environment.
  • Experienced in networking with community clergy and congregations, building strong relationships and community ties.
  • Deep understanding of and dedication to the hospice philosophy of care, emphasizing compassionate, patient-centered support.
  • Possession of a valid driver's license, access to a reliable and insured automobile, meeting state and organizational standards.
  • Previous hospice experience is advantageous, providing a background in end-of-life care and support services.

  • Proficiency in Spanish is beneficial, though not mandatory.
