About the position

Halcyon Home is seeking a part-time Hospice Chaplain to provide compassionate spiritual care to terminally ill patients and their families in Houston, TX. The role involves assessing spiritual needs, providing counseling, and facilitating visits from local clergy. The Chaplain will also be an integral member of the Interdisciplinary Group/Team, contributing to patient care planning and quality improvement processes.

Responsibilities

• Deliver compassionate spiritual care to terminally ill patients and their families in their homes.
• Assess spiritual needs and provide counseling to address those needs.
• Facilitate visits from local clergy, pastoral counselors, or other supportive individuals.
• Actively participate as a core member of the Interdisciplinary Group/Team (IDG/IDT).
• Attend IDG/IDT meetings and participate in the patient care planning process.
• Provide support for the spiritual needs of patients and families as evidenced by the Plan of Care (POC).
• Use effective interpersonal relations and communication skills to achieve desirable outcomes.
• Meet mandatory continuing education requirements of the Hospice.
• Promote the Hospice's philosophy and administrative policies to ensure quality of care.

Requirements

• Master of Divinity preferred.
• One year of experience as a Chaplain in a healthcare institution preferred.
• Working knowledge of community spiritual and other resources.
• Good interpersonal skills.
• Reliable transportation and current auto liability insurance.
