About the position
The Hospice Chaplain at Trinity Health At Home provides essential spiritual care to patients, families, and colleagues, focusing on spiritual distress, grief support, and crisis intervention. This role is integral to the healthcare team, ensuring that the spiritual needs of those served are met while promoting healthy communities and sacred environments. The Chaplain will develop and participate in spiritual care programs and collaborate with faith leaders to enhance the spiritual support provided to patients.
Responsibilities
- Deliver and facilitate transformative Spiritual Care experiences to patients, residents, participants, visitors, and colleagues.
- Honor and respect the religious beliefs, practices, and values of those served within the healing process.
- Implement systems to assess religious and spiritual needs, including ethics consultation, and provide appropriate interventions or referrals.
- Document professional Spiritual Care assessments and interventions in the electronic medical record, noting outcomes and plans of care.
- Plan and implement personal self-care and formation programs to maintain a healthy work-life balance.
Requirements
- Master's degree in theology, Religion, Spiritual Care, or Spirituality, or equivalent combination of education and experience.
- 1 to 3 years of Spiritual Care experience, including CPE training, is required.
- Chaplaincy or Spiritual Care ministry experience is preferred.
- Board Certified Chaplain (BCC) strongly preferred.
