Responsibilities
- Provide personal care and light housekeeping services to terminally ill patients.
- Participate as a member of the Hospice Interdisciplinary Team (IDT).
- Observe patients and report/document observations and care performed.
- Provide timely documentation of services based on the patient's care plan.
Requirements
- High school diploma or GED.
- Completion of a training program approved by the New Jersey Board of Nursing.
- Certification as a Hospice Aide/Homemaker from an approved program.
- Valid driver's license and reliable transportation insured according to New Jersey requirements.
- One year of direct patient care experience in an institutional or home care setting.
