About the position
The Hospice Clinical Triage Team Lead is responsible for overseeing after-hours, holiday, and weekend call coverage for the clinical support team. This role ensures appropriate staffing, manages clinical support operations, and provides high-quality, patient-focused care. The Team Lead is also responsible for documentation and coordination with on-call nurses to meet patient needs effectively.
Responsibilities
- Provide after-hours, holiday, and weekend call coverage for the clinical support team.
- Maintain appropriate staffing levels to meet business needs and ensure customer satisfaction.
- Oversee and support clinical support operations, ensuring adherence to triage protocols and best practices.
- Step into the role of a Clinical Support Specialist in emergent situations as needed.
- Handle calls from patients and family members, assessing concerns and ensuring high-quality care delivery.
- Document calls accurately within the electronic medical records system and process associated workflows.
- Coordinate with agency on-call nurses to deliver timely nursing care and schedule assessments as required.
- Maintain education on updates and processes related to electronic medical records and regulations.
- Exhibit exemplary communication skills when assessing or educating patients and caregivers.
- Facilitate orientation of new personnel as assigned.
Requirements
- Must be a licensed Registered Nurse in good standing with no restrictions in all states of practice.
- Compact licensure preferred.
- Two (2) years' experience as a Registered Nurse with at least one year of direct patient care in a hospice, home health, or oncology setting.
- Previous leadership experience in Hospice of at least one (1) year preferred.
