The Hospice Community Care Liaison (LPN) at AMG Hospice of Arizona is a full-time role focused on connecting with patients and families to educate them about hospice care options. This position combines clinical expertise with marketing and outreach efforts to promote hospice services, ensuring that patients receive the necessary resources and support during challenging times.

Responsibilities

• Building and nurturing relationships with healthcare providers, community organizations, and referral sources.
• Promoting hospice services to increase awareness and build trust in the community.
• Meeting with potential patients and their families to discuss hospice services with sensitivity and compassion.
• Explaining hospice care, eligibility criteria, and benefits to support informed decision-making.
• Conducting preliminary evaluations to ensure potential patients meet hospice eligibility requirements.
• Coordinating with clinical teams to facilitate a seamless transition into hospice care.
• Delivering presentations to healthcare professionals, families, and community groups about hospice care and its value.
• Acting as a resource for patients, families, and referral sources, answering questions and providing guidance.
• Partnering with clinical and administrative teams to align outreach efforts with patient care priorities.
• Staying up-to-date on industry trends and regulations to enhance outreach and education efforts.

Requirements

• Current LPN license in Arizona.
• At least 1 year of experience in hospice care is required.
• Strong interpersonal, communication, and presentation skills.
• Passion for providing compassionate care and building meaningful connections.
• Experience in marketing, community outreach, or patient education.
• A valid driver's license and reliable transportation for community travel.
