About the position

The Hospice LPN position at Cardinal Hospice Care involves providing skilled nursing care to terminally ill patients in their homes under the supervision of a Registered Nurse. The role emphasizes collaboration with healthcare teams to ensure high-quality patient care, pain management, and emotional support for patients and their families. Responsibilities

• Provide skilled nursing care to patients in their home under the supervision of a Registered Nurse.
• Collaborate with the RN Case Manager, physician, and interdisciplinary group to ensure comprehensive patient care.
• Assess and reassess pain, utilizing appropriate pain management techniques.
• Educate patients and families regarding pain management and symptom control.
• Carry out pain management and symptom management plans to maintain patient comfort.
• Respond to the physical, mental, social, and emotional needs of terminally ill patients and their families.
• Demonstrate knowledge of medications and their correct administration based on patient age and clinical position.
• Perform patient care responsibilities in a safe environment, reducing the likelihood of medical errors.
• Adhere to all Hospice policies, payroll, billing, and documentation procedures.
• Participate in mandated activities, including inservices and QAPI committee as requested by the Clinical Nurse Manager.

Requirements

• CPR Certification
• NC LPN License
• Driver's License
• Associate's degree in nursing
• Experience in case management
• Ability to develop and implement care plans
