As a Hospice Licensed Practical Nurse (LPN) at Gentiva Hospice, you will play a crucial role in providing compassionate care to patients in their homes during some of the most challenging times of their lives. Reporting directly to the Administrator or Executive Director, your primary responsibility will be to deliver direct patient care services to hospice patients while also offering supportive care to their families. You will manage a reasonable caseload, ensuring that each patient receives the attention and care they need. Your duties will include evaluating patient and family needs, assisting in the preparation and execution of individualized care plans, and documenting all findings and changes in the patient's condition. You will also be expected to communicate effectively with the Registered Nurse, interdisciplinary team, and referring physician, ensuring that everyone involved in the patient's care is informed and aligned. Regular attendance at staff meetings will be required to stay updated on best practices and team objectives.
