About the position

As a Hospice Licensed Practical Nurse (LPN) at Gentiva Hospice, you will play a crucial role in providing compassionate care to patients in their homes during some of the most challenging times of their lives. Reporting directly to the Administrator or Executive Director, your primary responsibility will be to deliver direct patient care services to hospice patients while also offering supportive care to their families. You will manage a reasonable caseload, ensuring that each patient receives the attention and care they need. Your duties will include evaluating patient and family needs, assisting in the preparation and execution of individualized care plans, and documenting all findings and changes in the patient's condition. You will also be expected to communicate effectively with the Registered Nurse, interdisciplinary team, and referring physician, ensuring that everyone involved in the patient's care is informed and aligned. Regular attendance at staff meetings will be required to stay updated on best practices and team objectives. Responsibilities

  • Provide direct patient care services to hospice patients.
  • Offer supportive care to the patient's family.
  • Evaluate patient and family needs and assist in the preparation and execution of the plan of care.
  • Report findings or changes in the patient's condition to the Registered Nurse, interdisciplinary team, and referring physician.
  • Document all reports and findings accurately.
  • Attend staff meetings to discuss patient care and team objectives.

Requirements

  • Compassionate, kind, and empathetic nature towards patients and families.
  • Licensed Practical Nurse (LPN) license to practice in the state of requested employment.
  • 1+ years of patient care experience in hospice, home health, ICU, geriatric, or oncology preferred.
  • Experience in hospital settings such as Staff LPN, Staff LVN, Med Surg, PACU, Float pool, ER, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, nursing home, or travel is preferred.
  • Current driver's license and automobile insurance.
  • Current CPR certification.

Nice-to-haves

  • Experience in hospice care or palliative care settings.
  • Familiarity with electronic health records (EHR) systems.
  • Ability to work independently and as part of a team.
