As a Medical Social Worker at Hospice and Palliative Care of California (HPCC), you will play a vital role in delivering high-quality medical social services to patients and their families. In this role, you will conduct comprehensive psychosocial assessments within five days of a patient's admission to the hospice program. This assessment will cover emotional, social, financial, and environmental resources, allowing you to identify appropriate psychosocial problems and develop interventions and goals tailored to the patient's plan of care. You will actively participate in Interdisciplinary Group (IDG) meetings, contributing to the development of care plans and assisting the team in understanding the significant social and emotional factors related to terminal illnesses. Your responsibilities will also include observing, recording, and reporting changes in the patient's emotional and social factors, maintaining written clinical records, and evaluating the effectiveness of social work interventions. Providing psychosocial and emotional support to patients and families is a crucial aspect of your role, as is participating in discharge planning and assessing the coping abilities of family members during the dying process. You will help patients utilize family and community resources, ensuring they receive the support they need during this challenging time.

Responsibilities

  • Conduct a psychosocial assessment as part of the comprehensive assessment within 5 days of admission to the hospice program.
  • Participate in IDG meetings and the development of the patient's plan of care.
  • Assist the IDG in understanding the significant social and emotional factors related to a terminal illness.
  • Observe, record and report changes on the patient's emotional and social factors affected by the patient's illness.
  • Maintain and submit written clinical records, including the initial evaluation, care plan, and daily notes.
  • Evaluate the patients and family's response to, and effectiveness of, the medical social work intervention.
  • Provide psychosocial and emotional support to the patient/family.
  • Participate in IDG and QAPI meetings and activities.
  • Participate in discharge planning, as needed.
  • Assess the patient's/family members' ability to cope with the patient's dying.
  • Help patients utilize the resources of their families and the community.

Requirements

  • Master of Social Work degree from a school accredited by the Council on Social Work Education and clinical experience relevant to the psychosocial needs of patients and families OR Baccalaureate degree in Social Work from an institution accredited by the Council on Social Work Education; one (1) year of Social Work experience in a healthcare setting required and will be supervised by an MSW.
  • Hospice experience preferred.
  • Must be a licensed driver with an insured automobile in good working order.
  • Ability to communicate well, both verbally and in writing in English and Spanish.
