About the position
The Hospice Medical Social Worker at AdventHealth is responsible for planning, implementing, and organizing social work resources to address mental health issues for patients. This role involves assessing patients' needs and linking them to community resources, ensuring optimal patient outcomes through collaboration with an interdisciplinary team.
Responsibilities
- Assess patient's physical, psychosocial, cultural, and spiritual needs through observation, interviews, and review of medical records and clinical data.
- Evaluate patient's response and intervene to ensure optimal patient outcomes.
- Develop and implement patient plans of care and observe outcomes in accordance with social work standards.
- Demonstrate competency in a variety of therapeutic and diagnostic interventions.
Requirements
- Master of Social Work (MSW)
- Current Basic Life Support (BLS) certification from the American Heart Association.
