About the position

The Hospice Nurse Liaison is responsible for developing and maintaining relationships with referral sources, primarily hospitals, to facilitate hospice care admissions. This role involves marketing the hospice services, educating healthcare professionals and families about hospice benefits, and coordinating patient admissions with the interdisciplinary team. The position requires strong communication skills and a compassionate approach to foster trust and awareness of hospice services within the community.

Responsibilities

• Develop working relationships with current and potential referral sources, focusing on hospitals.
• Establish contractual agreements with hospitals for GIP and Respite Care as needed.
• Implement marketing solutions and clinical coordination for potential hospice patients.
• Educate healthcare professionals, patients, and families about hospice services.
• Attend community events to promote hospice awareness.
• Coordinate patient admissions with the interdisciplinary team and Intake Department.
• Track and trend admission and discharge data for assigned locations and facilities.
• Negotiate service contracts with hospitals for GIP and Respite Care when necessary.
• Maintain assigned referral source goals and respond to customer requests and concerns.

Requirements

• Bachelor's degree in nursing, marketing, business, or a related field (preferred).
• Minimum of 1 year hospice experience (preferred).
• 2 years sales experience in healthcare services or a combination of experiences and education.
