About the position

The Nurse Practitioner (NP) is responsible for conducting face-to-face encounters with hospice patients to assess their eligibility for continued hospice care. This role involves performing evaluations prior to the start of the third benefit period and subsequent re-certifications, ensuring compliance with hospice care regulations and maintaining communication with the clinical team.

Responsibilities

  • Perform face-to-face encounters with hospice patients prior to the start of the 3rd benefit period and all subsequent re-certifications.
  • Gather and document clinical findings to determine continued hospice eligibility.
  • Share clinical findings with the hospice medical director.
  • Attest in writing that the face-to-face encounter was performed and findings were provided to the certifying physician.
  • Communicate with the Clinical Director and RN Case Manager regarding pertinent details of the patient's disease process and outcomes of visits.
  • Make patient consults and visits as appropriate and requested.
  • Attend interdisciplinary team meetings as appropriate.
  • Provide in-service education as requested.
  • Maintain sound privacy and security practices and take corrective action in case of breaches.

Requirements

  • Current License as Nurse Practitioner in the State of Alabama.
  • Minimum of one (1) year medical/surgical experience.
  • Hospice/Home Health experience preferred.
  • Current, valid Alabama Driver's License.
  • Proof of automobile insurance coverage.
  • Valid CPR certification.

Nice-to-haves

  • Experience in Surgery.
