We are currently seeking a Nurse Practitioner in the Chula Vista area who shares our passion for providing dignified, compassionate end-of-life care. This person will be covering the South County San Diego- 8 fwy to the border areas. Shift Schedule: Per Diem (PRN), As needed for face to face visits.

Responsibilities

  • Performs comprehensive nursing assessments
  • Assesses signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action
  • Initiates plan of care, all services provided to patients/families in all settings
  • Provides direct Face to Face visits to referred and existing patients contacts physician for patient updates
  • Maintains knowledge of state and federal regulations and JCAHO standards (if applicable)
  • Participates in QAPI program

Requirements

  • Graduate of an accredited school of Advanced Practice Registered Nursing
  • End of Life Nursing Education Program (ELNEC) - to include all 8 modules must be completed within 18 months of employment with Silverado Hospice
  • Current Advanced Practice Registered Nurse License within the state of employment
  • MSN or Doctoral Degree required
  • Certification in Hospice and Palliative Nursing desired
  • Valid driver's license, auto insurance, reliable transportation, and willingness to drive to patient's location
  • Must be able to lift 50 pounds
  • Must clear a criminal background check, physical and drug screening
  • You may be required to work outside of your regular hours depending on business needs
