About the position
The Hospice Nurse Practitioner (PRN) at Heart'n Soul Hospice is responsible for providing high-quality end-of-life care to patients in the community, particularly focusing on underserved populations. This role requires a strong background in nursing and advanced practice, with an emphasis on palliative care and hospice services.
Responsibilities
- Provide comprehensive care to hospice patients in their homes or facilities.
- Conduct assessments and develop care plans tailored to individual patient needs.
- Collaborate with interdisciplinary teams to ensure holistic patient care.
- Educate patients and families about end-of-life care options and resources.
- Manage medications and treatments in accordance with best practices in palliative care.
Requirements
- Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
- Current State license as a Registered Nurse and as an Advanced Practice Nurse (APN).
- Current Driver's License and reliable transportation with valid auto liability insurance.
- Two years of experience as an APN in a clinical care setting, preferably in hospice or home health.
- Comprehensive knowledge of pharmacology, palliative care, and end-of-life care.
