About the position

The Hospice Nurse Practitioner role involves providing medical care and support to hospice patients in their homes. The position requires participation in patient assessments, treatment recommendations, and collaboration with an interdisciplinary care team to ensure the medical needs of clients are met. The role emphasizes effective communication, patient education, and adherence to hospice care philosophy.

Responsibilities

  • Participate in the decision to recertify and discharge patients to/from Hospice.
  • Visit patients in home to assess recertification appropriateness for hospice and make treatment recommendations.
  • Write treatment orders as appropriate.
  • Work with the interdisciplinary care team to assure that the medical needs of the client are met.
  • Document recertification of a patient's eligibility in the Hospice program.
  • Cooperate with the attending physician to develop and update the plan of care.
  • Serve as a resource to hospice staff, patients, families, and attending physicians regarding pain and symptom control measures.
  • Attend IDT meetings as required.
  • Be available to interdisciplinary team members who have questions regarding the prognosis or care of Hospice patients.
  • Provide educational in-services to physicians, facilities, and the community at large.
  • Make routine home nursing visits as needed.
  • Conduct periodic chart reviews.
  • Perform patient/family teaching; utilize teaching tools and handouts; document all teaching and patient response.

Requirements

  • Board-licensed nurse practitioner, licensed to practice in NJ and PA.
  • Experience in family practice, oncology, or hospital practice.
  • Knowledge of the hospice philosophy of care and the delivery of medical care at home.

Nice-to-haves

  • Communicate effectively orally and in writing.
  • Assess patient's prognosis with regard to recertification of terminal illness.
  • Provide leadership and guidance to the Hospice team.
  • Ability to operate an automobile and enter/exit it.
  • Represent Hospice philosophy of care to the medical community.
  • Participate in educational seminars and programs as appropriate.
