About the position

Vitality Hospice & Palliative Care is seeking a dedicated Registered Nurse Case Manager (RNCM) to provide exceptional nursing care to patients in a hospice setting. This full-time position operates Monday through Friday, focusing on improving the quality of life for patients and their caregivers. The RNCM will work collaboratively with an interdisciplinary team to develop and implement individualized care plans, ensuring compliance with documentation and care standards.

Responsibilities

• Provide professional nursing care to patients.
• Complete admission, initial comprehensive, and ongoing assessments of patients.
• Initiate, develop, and update individual care plans according to patient needs.
• Collaborate with the hospice physician and interdisciplinary team to implement interventions.
• Manage and supervise home health aides, social workers, and chaplains.
• Educate patients and caregivers as appropriate.
• Uphold high standards of compliance regarding documentation and patient care requirements.
• Contribute to the continuous improvement of care quality.

Requirements

• Current New Jersey Registered Nurse (RN) License
• Minimum one year of Registered Nurse (RN) experience
• Valid driver's license with reliable transportation and liability insurance
• Strong communication and interpersonal skills
