About the position

The Hospice Nurse, RN at 3HC Home Health and Hospice is responsible for coordinating all hospice services in assigned counties, ensuring high-quality, compassionate care for patients and their families. This role emphasizes creating positive experiences, maintaining effective communication, and ensuring compliance with hospice guidelines and documentation requirements.

Responsibilities

• Creates positive experiences for internal and external customers that will meet their expectations.
• Provides comfort to patient/family to facilitate quality of life and death with dignity.
• Maintains current electronic documentation and downloads daily, ensuring complete documentation supporting Hospice guidelines & COPs.
• Ensures that the physician and/or Hospice Medical Director is involved ongoing with the patient's care.
• Maintains regular communication with supervisor regarding patient problems and care coordination.
• Ensures that 3HC can bill for services appropriately.

Requirements

• Licensed as a Registered Nurse in good standing in North Carolina.
• Prefer Home Health/Hospice Experience.
• Minimum of one year experience in Acute Care Setting.
• Current CPR certification.
