About the position

We are seeking a driven and compassionate Admissions Hospice Nurse to join our team in a PRN (per visit) role. In this position, you will provide high-quality care to patients with life-limiting illnesses, ensuring comfort and dignity from the very start of their services. You will work closely with patients, their loved ones, families, and interdisciplinary teams to deliver holistic care tailored to individual needs.

Responsibilities

• Conduct evaluations and assessments of a referred person's appropriateness for hospice care.
• Assess and communicate to the care team a patient's physical, emotional, and spiritual needs.
• Develop and implement personalized care plans in collaboration with the healthcare team.
• Administer medications and treatments as prescribed.
• Provide skilled nursing interventions as necessary.
• Assess patient conditions and report to the healthcare team promptly upon admission.
• Educate patients and families about end-of-life care options and support services available.
• Offer emotional support and counseling to patients and their families during difficult times.
• Ensure compliance with all regulatory requirements and maintain accurate patient records.
• Complete charting in EMR and other systems as needed in a timely manner.

Requirements

• Valid nursing license in the state of practice.
• Experience in hospice care, critical care, or emergency room settings is highly desirable.
• Strong communication skills to effectively interact with patients, families, and team members.
• Ability to work independently while also being a collaborative team player.
• Compassionate approach with a focus on patient-centered care.
• Willingness to undergo any necessary training related to hospice care practices.
• Must pass background check and have a Level 1 Finger Print Card.
