About the position

The Registered Nurse (RN) position at Empath Health involves providing compassionate, high-quality care to patients in a hospice setting. The role requires collaboration with patients, families, and interdisciplinary teams to develop and implement individualized care plans, ensuring that the needs of terminally ill patients are met effectively. This part-time position is specifically for weekend nights, emphasizing the importance of flexibility and commitment to patient care during critical hours.

Responsibilities

  • Obtain patient history and perform age-specific physical, psychosocial, and spiritual assessments.
  • Initiate appropriate referrals, interventions, and collaborations with patients, families, and the interdisciplinary team (IDT).
  • Identify patient/family goals and develop a Plan of Care in collaboration with the patient, family, IDT, and physician.
  • Evaluate progress towards goals and update the Plan of Care as needed according to policy and guidelines.
  • Provide care coordination and collaboration between the patient, family, IDT, physician, and partnering facilities.
  • Utilize critical thinking, judgment, and ethical principles for problem-solving and effective collaboration in clinical situations.

Requirements

  • Associate's degree (A.A.) or equivalent from a two-year college or technical school.
  • Minimum of six months to one year of related experience and/or training, or equivalent combination of education and experience.
  • Valid Florida license as a Registered Nurse.
  • Valid Florida State Driver's License.
  • Valid CPR certification from the American Red Cross or American Heart Association.
  • Minimum of 1 year of current active nursing experience, preferably with terminally ill patients or those with life-threatening illnesses.
  • Prefer one year of experience in an acute or post-acute care setting.
