About the position

The Hospice Registered Nurse Case Manager is responsible for providing high-quality, personalized care to patients in hospice settings. This role involves assessing patients' needs, developing individualized care plans, coordinating with a multidisciplinary team, and ensuring effective communication regarding patient conditions. The position requires a commitment to quality improvement and participation in educational activities.

Responsibilities

• Perform an initial, comprehensive assessment of patients.
• Document observations, clinical findings, problems, skilled interventions, goals, and discharge plans.
• Initiate and regularly re-evaluate and revise the plan of care in consultation with the Clinical Team Manager.
• Assess the need for services from other team members (PT, OT, ST, MSW, HHA).
• Provide and document skilled care on all visits, including skilled observation and teaching of the patient and/or family.
• Obtain and document physician orders.
• Perform Home Health Aide supervisory visits as per regulatory requirements.
• Coordinate care with the Clinical Team Manager, physician, and other home care team members, informing them of significant changes in the patient's condition.
• Submit weekly visit schedule of assigned patients.
• Collaborate with Clinical Team Manager to address scheduling needs.
• Perform re-certifications, resumption of care, transfers, and discharges as requested.
• Attend and actively participate in clinical team multidisciplinary patient conferences.
• Participate in agency-sponsored in-service education and quality improvement activities.
• Assist in the development of agency protocols, procedures, and policies as requested.
• Assess, develop, organize, and deliver teaching materials for assigned home care patients.

Requirements

• Current California RN license
• Minimum of one year of nursing experience
• Current CPR Certificate
• Current California driver's license and automobile insurance
