About the position

The Hospice RN Care Manager at CareFor is responsible for providing compassionate and comprehensive care to patients in their homes during their end-of-life journey. This role involves assessing, planning, and implementing individualized care plans while supporting and educating patients and their families.

Responsibilities

• Conduct initial and ongoing comprehensive assessments of patients' needs.
• Document findings and develop personalized care plans.
• Implement and document care plans to ensure quality and continuity of care.
• Educate patients and families on disease processes and self-care techniques.
• Coordinate with the interdisciplinary team to prioritize care needs.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare clinical and progress notes reflecting patient progress.
• Maintain effective communication with patients, families, and healthcare professionals.
• Provide on-call services to support patients and families during emergencies.
• Participate in professional growth and hospice-sponsored in-service programs.

Requirements

• Current RN license in Texas or in accordance with the Nurse Licensure Compact.
• 2 years of nursing experience preferred, with 1 year required.
• Experience in hospice or palliative care is preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of an interdisciplinary team.
• Valid driver's license, insurance, and reliable transportation.
