The Hospice Registered Nurse (RN) Case Manager plays a crucial role in providing compassionate care to terminally ill patients and their families. This position is responsible for pain management and symptom control, ensuring that care is delivered according to the established plan by the interdisciplinary team, attending physician, and Hospice Medical Director. The RN will operate primarily in the Marietta, GA area and surrounding communities, making home visits to assess the needs of patients and their families. This role requires a deep understanding of hospice care principles and the ability to communicate effectively with both patients and healthcare providers. In this position, the RN will conduct thorough assessments during scheduled and after-hour visits, documenting all findings, nursing interventions, and outcomes. Coordination of care is a key responsibility, requiring collaboration with the interdisciplinary team, attending physician, and other healthcare providers to ensure that all aspects of the patient's care are addressed. The RN will also facilitate communication among team members and ensure continuity of care across various settings, including long-term care facilities. The RN will supervise the care provided by Hospice Aides and Licensed Practical Nurses, ensuring compliance with the established plan of care. Participation in on-call rotations is expected, providing care after office hours, on weekends, and holidays. The RN will also monitor patient eligibility for hospice services and complete necessary documentation for certification and recertification.
