About the position

The Hospice Registered Nurse (RN) Case Manager plays a crucial role in providing compassionate care to terminally ill patients and their families. This position is responsible for pain management and symptom control, ensuring that care is delivered according to the established plan by the interdisciplinary team, attending physician, and Hospice Medical Director. The RN will operate primarily in the Marietta, GA area and surrounding communities, making home visits to assess the needs of patients and their families. This role requires a deep understanding of hospice care principles and the ability to communicate effectively with both patients and healthcare providers. In this position, the RN will conduct thorough assessments during scheduled and after-hour visits, documenting all findings, nursing interventions, and outcomes. Coordination of care is a key responsibility, requiring collaboration with the interdisciplinary team, attending physician, and other healthcare providers to ensure that all aspects of the patient's care are addressed. The RN will also facilitate communication among team members and ensure continuity of care across various settings, including long-term care facilities. The RN will supervise the care provided by Hospice Aides and Licensed Practical Nurses, ensuring compliance with the established plan of care. Participation in on-call rotations is expected, providing care after office hours, on weekends, and holidays. The RN will also monitor patient eligibility for hospice services and complete necessary documentation for certification and recertification. Responsibilities

  • Ensures timely and adequate delivery of hospice services to terminally ill patients and their families.
  • Assesses the total needs of the patient/family during home visits and documents assessments, identified problems, nursing interventions, goals, and outcomes.
  • Coordinates total patient/family hospice care under supervision of the Director and with the interdisciplinary team and attending physician.
  • Collaborates with the interdisciplinary team in developing, reviewing, and revising the clinical component of the patient/family plan of care.
  • Ensures continuity of care between patient/family, team members, ancillary providers, and attending physician.
  • Communicates patient/family/caregiver needs and ongoing nursing assessments through the interdisciplinary process.
  • Provides and documents continuing education of the hospice concept of care to patients and their families/caregivers.
  • Supervises the delivery of patient care provided by Hospice Aides and Licensed Practical Nurses, ensuring compliance with the established plan of care.
  • Facilitates the delivery of hospice services to patients in long-term care facilities and attends facility care plan meetings as appropriate.
  • Participates in on-call rotation for delivery of care after office hours, on weekends, and holidays.
  • Monitors patient appropriateness for hospice services and completes required documentation for certification and recertification.

Requirements

  • An active RN License in the state of application.
  • Valid CPR certification.
