About the position

The Hospice Registered Nurse Case Manager is responsible for providing comprehensive care to hospice patients, focusing on pain management and symptom control as outlined in the patient's plan of care. This role involves collaboration with an interdisciplinary team to ensure the delivery of high-quality hospice services to terminally ill patients and their families.

Responsibilities

  • Ensure timely and adequate delivery of hospice services to terminally ill patients and their families.
  • Assess the total needs of the patient/family during scheduled and after-hour home visits.
  • Document assessments, identified problems, nursing interventions, goals, and outcomes of interventions.
  • Coordinate total patient/family hospice care under supervision of the Director and with the interdisciplinary team.
  • Collaborate with the interdisciplinary team in developing, reviewing, and revising the clinical component of the patient/family plan of care.
  • Ensure continuity of care between patient/family, team members, ancillary providers, and the attending physician.
  • Communicate patient/family/caregiver needs and ongoing nursing assessments through the interdisciplinary process.
  • Provide and document continuing education of the hospice concept of care to patients and their families/caregivers.
  • Supervise the delivery of patient care provided by Hospice Aides and Licensed Practical Nurses.
  • Facilitate the delivery of hospice services to patients in long-term care facilities and attend facility care plan meetings as appropriate.
  • Participate in on-call rotation for delivery of care after office hours, on weekends, and holidays.
  • Monitor patient appropriateness for hospice services and complete required documentation for certification and recertification.

Requirements

  • Active RN License in the state of application
  • Valid CPR Certification
  • At least 1 year of experience in a hospice setting (preferred)
