About the position

Ennoble Care is seeking a full-time Hospice Registered Nurse Case Manager to provide skilled nursing assessments and care for patients in Newnan, GA, and surrounding areas. The role involves working as part of an interdisciplinary team to maximize patient comfort and health, while also supporting their loved ones. The RN Case Manager will be responsible for managing a patient caseload, coordinating care with physicians and other staff, and ensuring accurate documentation of patient information.

Responsibilities

  • Assume primary responsibility for a patient caseload, including assessing, planning, implementing, and evaluating nursing processes.
  • Initiate communication with attending physicians and other agencies to coordinate optimal care.
  • Maintain regular communication with the attending physician regarding patient needs.
  • Provide accurate and timely documentation in electronic medical records.
  • Participate in Interdisciplinary Group meetings and contribute relevant information.
  • Teach primary caregivers and provide direct care as needed.
  • Attend patient care conferences to discuss caseload issues and facilitate staff discussions.
  • Supervise and document the NAC every two weeks or as assigned.
  • Inform the Patient Care Coordinator of unusual patient/family issues.
  • Provide support at the time of death and during bereavement periods.
  • Participate in hospice orientation and in-service training programs.
  • Maintain familiarity with agency policies and state regulations.

Requirements

  • Possession of a current RN license in the State of GA.
  • 1-2 years of work experience as a professional nurse, preferably in hospice care.
  • Ability to assess and respond to the needs of patients and caregivers in varied settings.
  • Skilled in nursing practice and able to cope with emotional stress of caregivers.
  • Ability to make meaningful observations and write comprehensive reports.
  • Must be a team player supporting an interdisciplinary team concept.
  • Valid driver's license and reliable vehicle for year-round travel.

Nice-to-haves

  • Experience in palliative care or chronic care management.
  • Strong communication skills and ability to work independently.
